Extremely Poor phone system. I am calling for the 4th time this morning the first 2 times was transfer to a fax, when a live person did pick up after 10 minutes on hold I was immediately transferred back to the automated system. They need to straighten out the phone system especially when someone is calling to give important information to the nursing director.

My mom stayed at San Francisco Health Care Nursing Home at Grove until her passing. I am so grateful to all hard working, compassionate, and attentive staff there: CNAs, nurses, physical therapists. They were taking a good care of my mom. The building itself requires some renovation: all facilities are quite old (this is why I rated it 4 stars).

I feel truly blessed to have my husband received exceptional care and support from San Francisco Health Care and Rehab. This facility’s staff: nurses, CNAs and social workers kindness and dedication have made a significant impact on my husband’s well-being. I highly recommend this place to anyone wants the best care!

This is no place for hospice care: My husband is dying and when I visit him at the San Francisco Health Care and Rehab facility, I always leave with a pain in my heart. During my last visit I saw: him lying uncovered, heavily drugged, unresponsive, and at my last visit, with an open diaper lying in old loose poop, and a very red behind. When the CNA finally came she roughly rubbed his sore behind; he twitched and yelped at each rub (so did I.) I've experienced: always ignored as I stand at the desk at the nurses station, with backs turned on the swivel chairs, and at my last visit, after I asked for his nurse, adding that he needs help with him lying in poop, I received a loud aggressive vocal response from a CNA, seemingly a teenage. She said: "Oh yeah? He pooped before, that's two t

My aunt has been staying at this facility for over two months. This is an excellent facility because the nurses, therapists, and all the other staff are working so hard to care for the patients. The social workers are also very helpful. I would highly recommend this facility for someone who needs 24 hours care.
