San Diego Post Acute

Love the team at San Diego Post-Acute Center! I am familiar with other SNF's and assisted living facilities. I found SDPAC by refferal, toured it immediately, and it is excellent. They provide multiple levels of care all on one campus. The layout is pragmatic and feels safe, secure, and robust. Highly functional spaces and well organized care teams - with a nurse's station in each unit. Visitation has been a plus too. I've been able to visit during the day and the evening. The care team members are dedicated and patient with family and their guests - even when we are overwhelmed and struggling ourselves. Appreciate the spectrum of services, rehab, therapists, operations, nurses, and aides providing care and support for my own family member. I would highlight that both the admissions

If I could this place zero stars I would! Awful!! Completely negligent to elderly patients especially with dementia. My grandma was lost on numerous occasions upon me visiting they had no ideas where she was , one time found in a man’s room with the door closed. Not showered for multiple days, once I found objects in her diaper , had dirty diapers every time I went to check on her which was about daily. They had bruised her arms all up and down from grabbing her inside of guiding her from her waist. I actually ended up having to call 911 for help .

I selected this rehabilitation center for my mom, 87 years old, based on reviews. And I can tell that this facility is great. I am very happy with all process of admission and care. All nurses and aids are very nice, supportive, attentive, and accommodating to a patient needs. They are all helpful. I can have peace of mind knowing that my mom is in good care. I appreciate each and every one. Thank you from the bottom of my heart.

This place is an absolute nightmare. My husband spent four months there this year (2024). He had Alzheimer's and was treated horribly, over medicated and completely neglected. There was never a way to contact staff or to speak with a doctor. None of them knew what they were doing. I was lied to when I asked nurses how my husband was doing or inquired about medications. No one was accountable or professional in any way except for the physical therapists who were pleasant. My husband was still completely mobile and continent when he went in and totally unable to walk and was in diapers when he left a few months later. He was doing very well overall until he was alone in this horrible place. He died one month after that in hospice. I know he'd still be alive if he hadn't been left there. I

The staff members have been attentive to my uncle's needs and care. They are polite and professional from the moment you walk in. Alexi has been extra helpful with getting my uncle in and out of his wheelchair - it's a tough task and we appreciate Alexi's help. Thank you to the entire team and staff for all you do.
