Physical Therapy At Dawn

Multiple claims were submitted to my insurance under this company for services I never received. These included items such as bifocals, which a physical therapy clinic does not provide. On some days, up to five claims were submitted in a single day. I have never visited this location or received services from them. I contacted the company multiple times requesting that they stop billing my insurance, but those requests were ignored. I ultimately had to dispute the charges with my insurance provider. The claims have since been resolved, but the process took hours to address and was extremely frustrating. I would strongly recommend others review their insurance statements carefully if they see claims associated with this business.
