My uncle was placed at Oak View from Orange Park Hospital. The first time I walked in I smelled urine. Staff was standing at nurse's desk looking at there phone's constantly. When I got him to assisted living the nurse said he was dehydrated. They got him fluids right away and he was better the next day. He said the staff at Oak View was not nice or kind to his needs. Words from his mouth " it was a hell hole". He had a bowel movement in the bed due to no assistance. As a veteran and and someone who contributed so much to his community this is a very sad place. I have so much more but I will leave it here for now.

Have a friend there and the conditions are terrible. Smells like urine walking through and cigarette smoke in patients room because people smoking to close to building. The nurse button for calling for help has been left on the floor multiple times and therefore my friend cannot get up to use bathroom and is in pain and cannot request anything. The people that operate this facility should be ashamed of the conditions and treatment. I’ll be contacting Morgan and Morgan to see if you will take things more serious than talking with whoever runs the place.

My best friend has only been there 3 weeks . Every time I go to visit her,which is everyday, her colostomy bag was leaking on her and her bedding. I had to go to the store and buy sheets and a mattress pad for the bed they have her on. If you care about your loved ones, DO NOT LET THEM COME TO THIS PLACE!

DO NOT USE THS FACILITY! DANGEROUSLY INCOMPETANT! I was there for approximately 6 weeks ending the last week in July 2025. Two weeks into the stay I got COVID as a new patient was brought in from hospital and there was inadequate quarantine. 4 weeks later i got severe diahrrea (completely liquid); immodium and pwerful anti- diahrreal meds didn't touch it. An inadequate stomach x-ray "revealed" a blockage or so the doctor, who never personally saw me, relayed through the nurses - they gave me myralax twice daily. They said they took stool samples though two were left on the TV counter in my room; CNAs reported to me none went out to the labs. They claimed I wasn't dehydrated though 9 days of 6-8 liquid stools per day, chaffed lips, and dry mouth and low urine output said different.

My father died in this facility after a mismanagement of pain medication. We brought in a hospice service to help after nurses fell far behind his managed care as a stage 4 cancer patient. This was due to lack of seeing a physician in house and getting medication on a timed schedule as instructed by his doctors. The hospice org called me the night before his death citing they had to change his medication because it wasnt being administered correctly by facility nurses. After death, the facility manager said the medications were just written down wrong; probably not knowing id already had contact with the hospice care before citing their fault.
