Oak Grove Health and Rehab

Very happy with my mother's care during her rehab stay of almost two weeks at Oak Grove. The staff worked diligently to help her recover from hip surgery and she was able to quickly return to her home. The nurses (Rebeca, Billy and Jemal) were especially helpful along with all the aides. The rehab center was also staffed with wonderful professionals who provided exceptional care.

From the very beginning, our family felt an incredible sense of support and care from this facility. Back in October, when my dad first reached out to explore availability for my mom following her hospital stay, the staff — especially the Director of Admissions — showed compassion, patience, and dedication that immediately put us at ease. Although my mom experienced complications in the hospital that delayed her release, we continued to feel supported throughout the process. Communication was consistent, reassuring, and thoughtful, which made an incredibly stressful time much easier for our family to navigate. Now that my mom is here, we truly feel it was the best decision for this next chapter in her healing journey. The facility itself is extremely clean, well maintained, and welcoming

My father needed to be discharged to a Skilled Nursing Facility after his stay in the hospital. I was grateful for Regina working diligently to ensure bed availability. My dad’s case worker ensured his outpatient care was set up prior to discharge. All in all, his care team delivered quality care. The facility itself is clean and well-maintained.

Oak Grove health and rehab is a very beautiful and clean facility. Management plus staff goes over and beyond to make sure their patients are well Taken care of! Families can leave here knowing that their loved ones are in good hands. They really aim to do their best and making it feel like a home away from home.

My mother in law was a resident here for 24 days after a fall and fracture to her cervical spine. My experience was that she was treated with care, respect, attentiveness and she had adequate physical therapy to improve her condition. I found the room to be clean, the staff to be friendly and the food seemed appealing and plentiful. My mother in law had more complaints than I did. She felt the evening care partners were often impatient, not as gentle as she would have liked and were longer to respond than the daytime workers. We visited daily, but not overnight so I can’t confirm or deny her impression. I rarely ever saw any other patients / residents and I thought that was unusual. All in all, it seems adequate for her needs and we were grateful for their support and care.
