Nursing Home

Grace Rehab facility is a joke...this place does not care about their patients or their families. My grandfather was there for a week and was treated horribly. There was little to no communication between the facility and us while he was there. He is a dialysis patient and Grace was supposed to arrange transportation for his appointments. He was stuck at dialysis for 5 hours after his treatment was completed without transportation. We were given misinformation on several occasions and when we went to pick him up he was in far worse shape than when he went in. Personnel would not help him use his phone or find his phone. He was stressed and anxious the entire time he was there. When other nurses evaluated him, he is severely dehydrated, unable to move, and has lost a significant amount of w

Overall my Mother who was transferred from the hospital in weak & worried condition and our family were very pleased with the care given, delicious meals, cleanliness of facility and friendly staff. We were impressed with several who seemed to go the extra mile and also help build her self confidence to be able to return home after 4 weeks of intensive therapy. There were regular visits from the Nurse Practitioner. Also, fun activities helped to pass time and to become involved.The Admission staff (esp. Susan W.)was also very helpful and available. We were blessed to have Grace Healthcare to take care of our Mother and we felt right at home when we first walked in the front door. Thanks again!

This is a horrible place to place your loved one. My dad was not kept clean. Hi bed area was filthy. One of the nurses did not observe to make sure he took his medication and I found narcotics in his bed between the sheets. The top people don't care when you express your concerns. They only seem to care about their employees not getting in trouble for the awful things they do. His clothing and decorations kept disappearing-The staff steals there. Finally, they allowed dad to become dangerously dehydrated and his potassium dropped dangerously low. I had them send him to the hospital where he was treated for two weeks. My dad stopped eating and was downgraded to hospice. Then I had him moved to a wonderful hospice facility where he started eating again. But thanks to the poor care Grace Heal

Oh God, this place is horrible. In the last 2 months my grandma has been hospitalized 3 times. They don't clean or change her. The staff lost her teeth and eye glasses. She was sent to the hospital soaking wet. This last time which was a couple days ago. I went to visit her and they had her laying in urine and bowl. Tells from the smell and the amount it was for hours. She had bed sores we knew nothing about. They put her in isolation for shingles that She didn't have. Took her to the ER the same night She was dehydrated, had a sever UTI that caused her heart to fail, She had fluid on her lungs. The doctor said if we had not brought her in She would not have made it through the night. Please DO NOT TAKE YOUR LOVE ONES HERE. IF YOU LOVE AND VALUE YOU FAMILY, DO NOT TAKE THEM TO GRACE HEALTH

I am a health care professional (Dialysis RD) and I have been working with patients living in Grace Health Care of Tucker. The level of professionalism and communication of the staff is outstanding. Their nurses always polite and ready to answer any questions. The dietitian Kristie is one of the best NH RDs I have met in my life. She always make sure to communicate with us to get lab updates from our dialysis clinic and to continue care accordingly.
