Northern Light Walk-in Care

Went there for a basic checkup and was given a form asking about my gender identity and sexual orientation. This had nothing to do with my basic checkup and I told them I wasn't comfortable filling it out. They were very hostile about this and insisted I answered the questions, and even when I thought I was past that after declining the nurse came in and continued digging asking me questions even though I had been very clear none of it was relevant to my checkup. If you bring your kids here, make sure you are with them through their entire visit. Very creepy that they're this interested and get hostile when you decline to answer questions about your personal life.
