Life Care Center at Wells Crossing

Personnel were friendly. Just visited my aunt. My Dad was in Life Care during COVID. tough time but nurses brought him around to the back at glass doors so his great grandkids and granddaughter could visit through the glass doors. That was truly appreciated.

Great facility with awesome staff. Very friendly and helpful and makes things feel like home. Excellent care and compassion. Nurses and aides are great and take great care of the residents. Front desk staff is very friendly and always helpful.

I was visiting my brother in the facility. It was very clean and nice. I was impressed with the sitting rooms for larger families to visit at the same time. The ice cream shop was nice. They were popping corn there at the shop as well. I liked everything to include areas to go outside.

I stayed at Life Care Center in July 2025. As a 51-year-old woman, I didn't anticipate a 19-day rehab stay. The overall experience was good. While there were a few minor issues, the nurses and CNAs were nice and attentive, nothing significant in that regard. My primary concern was the poor quality of the food; the only palatable option was the grilled cheese. Thankfully, I had family and friends who brought me meals almost daily and I feel bad for those who don't have that support. While I didn't encounter any roaches as mentioned in previous reviews, we definitely had an ant problem and they were large ones. My roommate and I killed several and one was even in my bed, which the OT lady also witnessed. We reported it and maintenance only sprayed inside the room but it didn't resolve the is

Looks beautiful in the common areas. But the rooms are so antiquated and nasty looking. No one ever responds to my calls. I don't even know if the push button thing is working. Im miserable in here. I came from Orange Park Medical Center from beautiful rooms to this. Not happy. Im needing rehab for a broken pelvis and want out of here. I came here to have help. At this point I had to wiggle myself out of bed and 1use a nasty walker to get to s nasty bathroom. Im actually doing all the moving myself. No help at all. Looks as though I'm going to work myself out of here on my own Dont send ur loved ones here.
