I had an authorization in which I call to make an appointment. The authorization had the address showed in Google 19634 Ventura Blvd STE 303 TARZANA 91356. I was scheduled for a month later since they were booked for that month I call. On my appointment date drove to the address only to find out they had moved or never provided me with their new address at 18607 Ventura Blvd Tarzana 91356, thankfully I wasn't far and left home early, in which I arrived on time. They were not able to assist me because they couldn't pulled my authorization in which they never notified me earlier on the date I had scheduled my appointment.
