Fountains Transitional Care Center

My mom was here recently after spending a few days in the hospital. To say the staff, culture and overall facility was amazing would be an understatement. Not only is the building absolutely beautiful but the staff treated my mom as if she were in a five star hotel while providing the best care. She was back on her feet in only a few weeks. The therapy team ensured she had everything she needed to be successful at home. I’m so thankful for the Fountains team for all their love and attention which allowed me to feel at ease while our family went through this difficult time.

I personally had an enjoyable stay at the Fountains. The Physical therapy and Occupational therapy was the best and Nursing was above and beyond wonderful. If you need rehab then may you have the opportunity to stay there and the Food was good as well ! Jodie the social worker was excellent and the aide's we're on point. Brandie the nurse went out of her way to help me. All the staff were excellent and helpful!

My mother has been in 4 care facilities. All very expensive and show stopping as far as first impression and planned tours. Her care was minimal at best. We then started popping in to several and found the Fountains The staff is caring and compassionate. They are working to give your loved ones the best care. I am pleased and thankful to find such a place for my mother.

I was a patient at The Fountains for several weeks and give it an excellent rating. The facility is new and beautiful, there are many amenities, and highly skilled staff. I found the staff to be friendly, knowledgeable, and helpful. They were always willing to go the extra mile. I received excellent care and therapy. I would recommend it without hesitation.

I highly recommend AGAINST choosing fountains as a place of care for your loved one. This agency over-sedated my grandfather who was only there for rehab regarding a catheter. They saw his age and basically tried to kill him off. He was very competent, and coherent before getting there. They disconnected his room phone on multiple occasions. Did not set his bed up properly, this was problematic due to the fact he needed to spit phlegm up when he coughs, he could have choked to death. They did not ensure he was hydrated while giving him multiple medications, did not check his blood pressure. Gave him a bed pan when they could have taken him to the bathroom. Lied about him receiving physical therapy on the weekend when PT does not work on the weekend. They lied about bathing him and so much
