Evergreen Center for Health & Rehabilitation

I was at Evergreen today visiting a family member who had arrived this morning. When got to his room, he was being cared for by the NP on duty and two other healthcare professionals. Within a half hour of their leaving, the programs coordinator came in to find out his interests and what he may like to participate in during his stay. He was served a hot lunch in a timely manner and follow-up staff promptly removed his tray and wiped down after. All of the staff was pleasant and professional.

The care and attentions I have observed in the Maple Lane have been top-notch!! We were there today (Thanksgiving) to see our loved one again and were thoroughly wow's by how wonderfully festive/homey the place was. It's those special touches that reaffirm our loved one is in the BEST place. Accolades to whoever makes that area seem as homelike as possible for residents and visitors.

My Mom has been here for around six months and it has been a pleasant experience thus far. The nursing staff is excellent, they have many activities and the director of activities is great. The one thing is the food is below average and needs improving.

It was the worst I have ever seen. They were not ready for him and then traumatized him by weighing him and putting him a wheel chair. Next day valuated him and came in and said do you want Hospice. No sit down and this is what happened and this is why he should be on hospice. After 2 days Christmas morning went in ad he was lying there with no blanket and one sock on and nothing on his legs. They said they were sorry. Thank God I was there from morning until night. He passed December 26th. For those three days once while I was there Jen came in to check on him. The care there is horrible. Do not place your love ones there.

Visiting my father. This place is always clean. No oder. People are super friendly. This is like a small community you walk in and everyone worker and residents talk to you. Bingo, music, arts and crafts everyday something for them. Nice to see everyone getting out of their rooms and talking to each other
