Bolivia Rehabilitation & Healthcare Center

I have been in this Healthcare Center for just over a month for PT after a fall. The staff here are very nice, helpful, and accommodating. Remember to treat others as you wish to be treated and you're golden. They administer PT according to you issue and progress, so it's motivating. They have various activities, spiritual programs, and other ways to encourage people to mingle. My only issue was with the food contractor. The paper with the meals don't always match. Sometimes others have different items than the others at the table, such as dessert or the main entree. I appears that the contractor is not timely so meals can be on time or late. This is the area that needs improvement. I also think that this facility is understaffed which effects both the staff and residents. Facil

This facility is so impressive to me! Clean, good care and most important the staff was everything I needed them to be and more in the care of my mother, who was dying and afraid to die. They allowed me to do what I needed to do during the process, which made my experience with my mom even that much more loving and caring. Thank you for being so awesome!! It made all the difference!!!

Bolivia has been the life line our family needs. The staff is always helpful and my husband's nurses are angels. They definitely found a calling, not a job. All the time I observe the compassion these nurses have for all the residents in his unit. The CNAs are personable and constantly attending the resident's needs. The maintenance staff cleans constantly, 7 days a week. They clean everything, not just the floors. I know the structure isn't new but it is well cared for. This facility has saved our lives for over a year. I thank God for them every day.

This has to be the worst nursing home there is , for starters my grandmother has been there for a week she fell last night and no one was there to help. When she first got there one cna was extremely rude and very unprofessional , my grandmother asked her for help and she said “she didn’t have time to do whatever she asked and she’ll take her time doing it”. To mention Friday afternoon when I went to visit my grandma she waited over an hour until someone finally got to her room. Let me just say this if you do not like your job or the job title for your job I suggest you don’t work in this field of work.

There's nothing good to say about this place. The building is old and stinks, and when you ask anyone for help they respond with "I don't know, I'm agency." What does this even mean? When you ask to speak to management, it doesn't seem like they have a clue about any nursing care or anything going on. The furniture here is covered in brown stains and there's pills scattered on the floor. The food isn't fit for a dog let alone a human being. Desperately needs a remodel and a new kitchen menu for sure. Don't take anyone here unless you want them to be covered in roaches inside this old awful place.
