1/9/2026 - Update..Since my last review management reached out to me within a few days and I truly appreciate how quickly they responded and took my concerns seriously. POSITIVE changes took place and mother feels better about the changes. Thank you, Debbie & Anthony 🙏🏼 Old review- I wish I could give the facility more stars. Debbie in administration is great but she's very busy. Maritza at the front desk is also great, but everyone else is difficult to reach. I read alot of the reviews. My mother has been there since February 2025, 1st floor was good, 2nd floor not so good, there is no team work. If a patient buzzes and the CNA assigned to that room is busy with another patient, the ones sitting in the station ignore the buzz and goof off on their phones. I am tired of trying to solve is

I transferred my 86 year mom from Chicago Insight Hospital to Bella Terra in Streamwood on 01/31/24. Dakota Burke the Director of the Bella Terra was so helpful in getting mom accepted into the facility. Dakota took my family for a tour of the facility before my mom got transferred over to Bella. Dakota when thru all of the guidelines of the facility very thorough. Dakota have some very good staff that been working very hard taking care of mom. I would like give a Great THANKS to May, Rose,Marcy and Maria( on the first floor) for all you have help and done for mom. And a special thank to Monette the social worker. And Marisa the front desk receptionist is always so friendly with a wonderful smile to all incoming visitors. Mom is so… happy at the facility and made a lots of new friends. I’

I was in there for along time and it was so bad...they leave you on the bed pan for hours and don't hanser the call lights and therapy was not happening I was left in my bed crying...Maria hurt me they definitely hurt me over and over didn't help me in any way...I and my husband asked them for help but never come.... Debbie didn't do anything to help they left me crying for help and weekends where the worse. Don't send your family here....and the food was very bad...mold on my bread and I had a bad experience here don't send your love ones here,!!!!

Elizabeth, Raymond and Eva are some of the best CNAs working there on the first floor. They are super helpful, kind hearted, and efficient. They’re a great team. Jen and Prescilla are amazing nurses and I’d trust them with my care plan. Management as in Debbie and Ann on the other hand not so much. I’d go into detail but I think the one star review is enough for now.

Check Medicare website. Their stats don't Lie End of Nov 2022, our 97 yrs old, WW2 Marine was living independent with assistance for showers, medications & meals. Everynight he called his financial institutions to check his account balances. On Dec 10th, he was transferred from CDH to Bella Terra for short term rehabilitation to regain his strength. He took his last breath on December 26th at 8:50pm. Although we are deeply sadden by his death with emotions extremely high, it is important that this review is posted to help validate the other families with similar experiences.
