My loved one was recently in your Sub-Acute for care. Our family was very pleased with the care provided to my dad by the nursing team and aides. A special thank you to Tamika, Tonya and Roxy who were incredibly attentive to his needs. They took wonderful care of my dad and were always kind, positive, and respectful, treating him with the dignity he deserves. I want to extend a heartfelt thank you to your Hospitality Director, Diana, for being so welcoming and attentive during his stay. She checked in with him daily to make sure he was doing well, eating his meals, and comfortable. If he needed anything, she was always quick to offer her help and support and communicate any questions to his care team. Omg ! Autumn lake is the best ! All of the staff on every shift are professional They smile, they say good morning, they always help you with anything you need. There just fantastic ! The front desk, greets you when you come in. There wonderful ! Doctors, Nurses, Social Worker, therapists, Sanitation crew, supervisors, aids. Etc. There all There when you need them. I want to thank Donte for finding Pete's phone. Wrigley cool lady. All of the aids are doing a professional job. I forget alot of There names, I met so many. I have nothing but very nice things to say about this facility. The facility is clean and pleasant. Rakayla Tania, Amanda, Tamaka, Sherry, (admin Courtney, and Michelle, these are the nurses and aides that take of me. Tania is my morning aide, from the first day I came to the facility she went over and above in which I am so thankful. She makes sure I am clean and comfortable. She is always there for all of her patients needs and concerns. Also Tamaka is my night aide she is just as wonderful! The same thing with her also.She makes sure I have everything I need before bed!
