My mom got great care at this facility! Some of these reviews were very scary but we experienced more kind and caring communication from the nursing management team. The wound care was exceptional! They have a wide variety of specialist so that mom didn’t have to leave the facility for every little thing. Mom is a picky eater so of course we would bring her snacks or a meal when we visited. But the dietician worked with her preferences and as the days went on and mom learned the facility and they learned her things just kept clicking in place. Then after the wonderful and very patient therapist worked with mom for about four weeks she was able to come home right in time for the holiday! The administrator and his team are wonderful! I thought that this facility would be suitable for my sister rehabilitation and therapy, I was so disappointed to learned that this facility standards, care and hygiene for their residents are the worse. My relative informed me that the facility did not have any adult diapers on hand and she was lying in a soiled diaper the entire day. The staff on duty excuse was that she just came on duty at 7pm and wasn’t aware of the issue. In the meantime, the previous staff did not attempt to resolve the issue with an alternative approach but to pass the problem onto the next scheduled staff. My relative had the same soiled diaper on since 11 am of that day. This place is always clean and smells fresh when I come by. The hallways look nice, and you can tell the staff works hard to keep it that way. The administrator is easy to talk to, professional, and really seems to care about the residents. The front desk staff are friendly and make you feel welcome right away. I’m really glad my loved one is here and being cared for by such a good team.

If only I could give this place 0 stars!!!! I am writing this review in the hopes that the management of this facility will see this and take action. When going to visit a family member for the first time months ago I was immediately taken back when entering the facility of the strong smell of urine in the hallways. At first I thought that it was just that day but after visiting numerous times after that I have concluded this is a normal occurrence. I don’t know how employees can work in that condition with the strong odor. Additionally, the floors in the hallways were extremely dirty and had wet puddles everywhere. They looked like they haven’t been cleaned in a long time with dried up marks of residue all over them. I have been to another autumn lake location before and never experienced

This place is disgusting. It’s dirty, smells of urine and the needs of the patient seems to be the least important thing on the list of things to do. 1 star is being generous.
